La rhinorrhée cérébrospinale spontanée est une maladie rare. Le plus souvent, elle est secondaire aux tumeurs cérébrales ou d’hydrocéphalie, rarement associée à un syndrome de la selle turcique vide.
Nous rapportons l’observation d’une patiente obèse qui a présenté un écoulement nasal gauche. L’exploration avait objectivé une selle turcique vide associée à une hypertension intra-crânienne idiopathique, compliquées de rhinorrhée cérébro-spinale non traumatique par une fuite ethmoïdale (localisation rare). Après traitement par Acetazolamide et des ponctions lombaires itératives, une amélioration clinique et radiologique ont été notées.
