Background:
The success of middle ear surgery needs adequate anaesthesia, which can be from general or local anaesthesia. General and local anaesthesia have their advantages and disadvantages. Otolaryngologist needs to consider the use of local or general anaesthesia which have a different approach for each case.

Pre-operative assessment is an important measure, considering size, delicate contents of the middle ear, and its unique location. Special consideration might be given to patients with comorbidities. The ideal target for every surgeon is to minimize bleeding. Considering the history of anaemia, cardiovascular disease, and hypovolemia can reduce the chance for hypotension. It is essential to avoid nausea and vomiting post-surgery, with minimal opioid usage for analgesia and adequate hydration. General and local anaesthesia is very different anaesthetic modality. Each of them has its advantages and disadvantages.
Conclusions:
Careful consideration is needed for anaesthesia in middle ear surgery, especially the patient health history, anaesthetic agents, and environment. Otolaryngologist needs to consider the use of general or local anaesthesia, which has a different case approach.
