Modern endodontic surgery is microsurgical. The microscope, endoscope, ultrasonic tools, microinstruments allow much less trauma. Flaps reflection is less invasive, ostectomy and osteotomy is much smaller which improves healing and treatment outcome. Key words: microscope, endoscope, haemostasis, flaps, isthmus
