Dynamics of Serum Lactate and Glucose Level with Coagulation Factors in Moderate and Severe Traumatic Brain Injury

Increased lactate and glucose serum level are consequences from metabolic abnormality happened after traumatic brain injury. This research aims to assess the difference between lactate and glucose serum level with coagulation factor in moderate and severe traumatic brain injury patients. This is observational research with cohort approach with research samples are all moderate and severe traumatic brain injury patients admitted to Wahidin Sudirohusodo General Hospital Makassar from January 2019 until we met sample quantity requirement, 30 patients. Statistical analysis was done based on measurement scale and research purpose. Statistic tests used were the Wilcoxon Rank Test and Mann-Whitney U test, in which the result was considered significant if p-value < 0.05. Research result shows that there is an increased lactate serum level in patient with traumatic brain injury, even though there was no significant difference between moderate and severe traumatic brain injury group in those 3 measurement time (day 1, day 3, and day 7; p = 0,77, p = 0,41 and p = 0,31 respectively). Moreover, we also found glucose level fluctuation in moderate and severe traumatic brain injury group (day 1, day 3, and day 7; p = 0,53, p = 0,97 and p = 0,33 respectively), even though there was no significant difference between mean glucose level. On the contrary, this analysis shows that patients with traumatic brain injury will have significant mean bleeding time between moderate and severe traumatic brain injury on day 1 with p = 0.02, day 3 and day 7 with p < 0.05. Nevertheless, the mean bleeding time between measurement in those 2 groups was within the normal range.
