The prerequisites for endodontic success includes a thorough knowledge of the root canal anatomy, careful interpretation of the angled radiographs, proper endodontic access cavity preparation, and exploration of the root canals. A highly variable root canal morphology is seen in maxillary premolars and it is quite rare to find three canals. However, it must be taken into consideration radio graphically and clinically during the endodontic treatment. The aim of the present article is to report a case about the diagnosis and management of three-rooted maxillary first premolar, with three independent root canals.
