Cardiac troponin elevation prognostic significance for acute aortic dissection; systematic review

Abstract

The current study aimed to assess the concentration of high-sensitivity cardiac troponin T (hs-cTnT) in patients with acute aortic dissection (AAD) at the time of admission to ascertain the use of hs-cTnT in forecasting the short-term prognosis of these patients. The study adhered to the PRISMA statement in this investigation. Six original studies that were published between the years 2015 and 2023 were included. A thorough literature search was conducted using PubMed and Scopus to locate all studies published until the year 2023 that looked at the outcomes of AAD patients who underwent troponin level measures. The terms “troponin” and “aortic dissection” were combined into the search parameters. Troponin was often positive in acute aortic syndrome patients, particularly when a high-sensitivity test was employed. Abnormal troponin levels were strongly associated with acute coronary syndrome-like ECG abnormalities and in-hospital diagnostic delay, but they did not appear to impact in-hospital death. Although one study suggested that hs-cTnT levels might be used as a bio marker for the risk of patient stratification with AAD type-A in the emergency department, more investigation is required to ascertain the relationship between these levels and long-term results.
