Makroscopic preliminary diagnosis and histopathological result may be different in peri-ocular masses, preliminary diagnosis should absolutely be confirmed by histopathology. Three cases with the preliminary diagnosis of basal cell cancer treated with surgery that had histopathological results from basal cell cancer were evaluated. Retrospectively with the diagnosis of basal cell cancer treated with surgery followed and histopathological results were evaluated in the three cases that were different. The lesions were present for varying periods of five months to one year. Direct surgical excision and primary suture or graft or flap for the reconstruction was performed in the three patients. The histopathological evaluation demonstrated chronic inflammatory findings and fibrovascular proliferation but rich lymphocytes in two cases and rich histiocytes in the other case. Preliminary diagnosis should absolutely be confirmed by histopathology. Biopsy option should be taken into consideration in suspicious cases.
