Selective use of peroperative cholangiography

A retrospective study of 497 consecutive patients who underwent elective surgical treatment for cholelithiasis and/or choledocholithiasis in Gazi University School of Medicine, Department of General Surgery from 1987 to 1990. Peroperative cholangiography (POC) was performed in 53 of 100 patients as further evaluation of common bile duct. Common bile duct exploration was performed in nine patients after POC. Of patients 47 were undergone to common bile duct exploration without POC. The rate of false negative and positive results of POC group were 1.9 % and 3.8 % respectively. Among the patients who did not undergo POC examination, the rate of false negative results was 8.5 % in common bile duct exploration.
Doubtless, POC is a valuable tool. If clinical, laboratory findings and surgical exploration remind a common bile duct stones, POC eliminates an unnecessary common bile duct exploration. Selective use of POC has been recommended to reduce operation time, morbidity and operation costs.
