A 6-year-old cross-bred cow presented with a unilateral swelling along the left side of the vulvar lips showed signs of straining during micturition and defecation. The mass was soft and did not manifest pain on palpation. The condition was diagnosed as a perineal hernia. A herniorrhaphy was performed, and no complications were reported in the follow-up period.
