ICT Solutions in Support of Chronic Disease Management

Abstract

The world’s population, in respect to demographic changes and ageing population, is increasingly impacted with chronic conditions that require continuous treatments, monitoring and support processes. Opposite to medical science and treatments available however, healthcare systems have been slower in pace to reform the incentive, reimbursement and care process models to meet this challenge. This is now beginning to show severe impacts on both quality of life of chronically ill, but also financial sustainability of healthcare delivery. Understanding this challenge, this paper will elaborate some of the early adopters of the chronic disease management programs, which are implementing innovative models and ICT solutions and showing impressive results in both increase of quality of life while at the same time controlling the cost of care delivery.
